Their mission works by creating a moral dilemma for whomever is receiving the free medical attention. This is the same way bin Laden works. He builds schools, provides medical care and feeds people and in return they pledge allegiance to his cause. It's a very simple exchange that works very well. bin Laden is a missionary.
