The pipes for the self-draining cockpit haven't been installed correctly -
port drain is going to port outlet and vice-versa.

No. That's why hulls crack open. But, I guess you could file that
under "ballast problems."

I've seen this.

It was caused by a keel mounted mast filling up with water. When the boat
was heeled, the mast bent sufficiently to break the seal at the keel - and
so the water leaked out.

Is this what you had in mind?
