Can anyone suggest where this guy went wrong!

The lift was wrong from the start! Never lift without a spread rig for the
slings. The crane should have had a rectangle of steel which is attached
to the top lift point. The slings are attached to the corners of this steel
cradle, then the boat is to be lifted and will be secure as the straps of
the sling will not move from their lift points on the vessel.
