In what military do servicemen request medals?

In Kerry's military. He came back from his first shootout with a
scratch on his arm. He submitted his fire report and asked his
commander to put in the paperwork for a Purple Heart. His commander
refused and later on was dumbfounded when Kerry was awarded the medal.
