Hi, all,

My apologies for being off topic, but this is the fastest way to

OT - Medical stuff in MIA - ENT, Mammography and Colonoscopy

We're going to be in Miami for a while, to attend to some medical
stuff, before we head out to the Bahamas. Actually, there's some
possibility that we'll be in West Palm, so this applies to both
areas.

We need recommedations for an ENT who does nasal resections and
"roto-rooter" - sinus resections - surgeries.

Likewise, Mammograms and Colonoscopies.

If you have satisfactory experience with any of these proceedures
(or know other satisfied users) in either of those areas, please
respond directly to me skipgundlach at gmail dot com.

Thanks.

We now return you to your regular programming.

You've clearly been going to the wrong practitioner. A small
purgative, emptying your colon, no enema or douch, and enough feel-
good meds to have you semi-conscious, but a side effect of not
remembering a thing when it's over.

Midazolam is a wonderful drug.
You wouldn't believe the number of patients I've sent for tests who, when
asked how it was, actually deny they have had it and think they are still
waiting for it. They usually believe us when we show them the pictures of
the inside of their colon/stomach/whatever ;-)

The technique to avoid that is don't joke about the procedure not being as
bad as going to the dentist while the doctor is doing the procedure. It
tends to elicit a lot of laughing at the wrong moment.

Huum... for the nose job? West Palm Beach. Look, the place is loaded with
very rich people who have
very large noses. If you get the drift..

The colon ? Miami. There are lots of assholes in Miami so the doctors are
familiar with them.

As for the boobs? Have that done when you get to the Bahama's.

Where did Anna Nicole Whatshername go for medical help? That's right, the
Bahama's.

And she had two of the most beautiful boobs [ ok, so what if they were
fake ] in the world.

Just don't invite any family members to visit you while in the Bahama's
medical tent.

It just does not work out.

Roger !

Geez, you're usually such a nice guy.

I don't think this is very encouraging to Skip.

Wally, have you ever been poked like that?

I'd have to agree with Roger on this one.

A satisfactory experience would be to avoid it like the plague!

Roger .. I want to double, triple, .. multiply ..

GET THE COLONOSCOPY ..

I had a good friend, he was a very outdoors guy. Always hiking the Whites,
runner, basketball player,, built his
own house from scratch .. I mean he literally dug the cellar.

But he was chicken when it came to doctors.

So .. he let a small problem turn into a death sentence.

Now he is gone.

We can't be out sailing and arguing on the net if we are dead.

All you sailors .. get the tests!

Do it for your boat! And for yourself!

Absolutely not! I've watched a couple people go through the results of
not finding the problem early enough.

I didn't mean to worry Skip. The procedure is so safe that I assumed he
had already been briefed and knew that. My PO's experience was extremely
rare. It was a highly insensitive comment though.

There is another side to this.

I had a bladder tumor in 2005.
The doctors immediately used the "C" word and scheduled surgery to cut
it out.

It was benign. NOT cancer at all.

But it has taken nearly two years to fully recover from the surgery.
Actually, I doubt anyone ever does fully recover.

I'm sure it was a bummer, but the alternative of doing nothing could have
easily been terminal. I'm assuming you got multiple opinions... several
friends did that and saved themselves a lot of grief from the first
opinion... that's why they call it a medical "practice."

In fact a chap just down the dock from me had exactly this experience.
Urinary tract problems; scared to see doctor; eventual couldn't
urinate and had to go to the hospital; Surgery. We scattered his
ashes in the ocean some 3 years after he first noticed symptoms.
Doctor said *probable* that had he visited the doctor when he first
noticed problems that they could have, at the minimum, delayed his

Hi, all,

In another venue,

Don Greer wrote:
I am surprised no one has repsonded to your request for help. Without
any personal knowledge, I would suggest you contact Seth Thaller, MD
DMD. 305-585-5285 Dr. Thaller is Chairman of the Department of Plastic
Surgery at the University of Miami. He does have a backgorund as an
ENT surgeon.
It apears Dr. Thaller has a private practice as well as directing the
residency program. If you call him, be sure to investigate carefully
the advantages of seeing either Dr Thaller in his private office, or
seeing a resident in the training program.
Most plastic surgery programs now do a lot of breast reconstructive
surgery. Because of that they will know which surgeons and which
radiologists are doing good work on breasts, and with the proper
questions should be able to help you find the best possilbe help.
Good luck.

I said:

This has appeared in several different venues. Aside from the "watch
out for horror story" colonoscopy commentary, the other curiosity I
have found is that everyone either misunderstands, doesn't know what
one is, or assumes that mammography implies the need for breast
surgery.

A mammogram - a specialized X-ray of breast tissue - is an annual
event for most women, but you can't just walk into some place and get
one - you have to be referred. We've got the referral, but we need a
recommendation.

Thanks for the recommendation on the ENT lead...

And, now, I observe...

So, not having any responses, I assume that nobody who's seen this
either has had the procedure(s - a colonoscopy, mammogram or roto-
rooter/resection of nasal passages) or knows of someone who has, in
the Miami through West Palm area, and can recommend the practitioner
who did the work?

Thanks...

Thanks...

because of medical non-mistakes.

If you're worried about doctors making mistakes (and they are only
human, despite what they're taught in med school), don't go to one.

Like anything else, there are some very good doctors out there, and there
are some very bad doctors. Unfortunately, there aren't many good ways to
find out which is which.

Yes, there is. First resource (in fact i would recommend this to Skip)
is the state medical review board. Find out how many problems any
particular doc has had with the procedure you're considering, how many
times they've been sued, etc etc.

The next is to get a list of board-certified MDs in the specialty
you're interested in. Although it isn't a guarantee, some real lu-lus
get thru board certs, but at least you're not going to a doc who once
heard about your procedure over pizza & beer back when he was a
resident, and hasn't even read a magazine article about it since.

Remember, lawyers make more money from the American health-care system
than doctors do. And the gov't wants to keep it that way.

Some states have hospital comparison information available, such as is
starting to be the case in Cal.

Good line. And it would be truly scary if it were true.

One could quibble over accounting details, but the scary thing is that
lawyers *do* make more money from health care than doctors. For
example, check how many MDs are on insurance company payrolls, and how
many LLDs.

I sympathize with
the thought, however. There are too many law suits, including malpractice
suits.

Definitely.

OTOH there is too much malpractice. The question is, should your
health care risk (which is never zero, regardless of the procedure) be
turned into a lottery?

Another excellent resource, thanks.

It's over due for a discussion of how cruisers manage their doctors
visits, not to mention insurance.

Interesting point, especially when headed for an international
destination. Fortunately we've never needed medical care while on our
5 and 6 month east coast cruises but it could certainly happen.

More accurately: The lawyers pay the government to keep it that way.

As soon as someone comes up with something more powerful than what the
lawyers pay, the laws will change.

A few of the stories I've heard over the years indicated that
outpatient service in many places costs less than the co-pays we have
come to expect.

But, evac for major stuff is quite expen$ive! Guy on one of our BVI
trips jumped off of the other boat onto the dock and broke both ankles.
Airlift to St. Thomas for major surgery and thence back to England was
"not trivial". Seems the UK system covered most of his costs, but I
believe he was out-of-pocket for some. And at least one on this list
can relate the story of a heart attack requiring a trip back to the US.

DAN is good for some stuff, but I believe there's a plan through the
SSCA that sounded better when I investigated.

You bet. One of our neighbors had to be emergency airlifted out of
Haiti several years ago at a cost of around $10K.

I got a better question, Doc. Where in the oath does it say every doctor
has to be a millionaire on the backs of the patient victims of the health
care system? Is $600/hour fair, $1200? $2400? .....to see a doctor
educated at PUBLIC expense?

Why do cruisers, or any of the rest of us, have to "manage their doctors
visits" at all? What profitable diseases do doctors prevent, at their own
expense?

Can we afford to find a cure for cancer? Of course not....no more than for
any other real or contrived (HIV) diseases.

???

The only docs educated "at public expense" AFAIK is in the military,
and they fulfill a very rigorous set of obligations to pay it back.

Most MDs I am aware of enter practice with anywhere from $150K to
$500K of debt (very very very few with less, many with more).... often
to the gov't... also often to medical education foundations.

If you got a serious hate-on against doctors, that's a very simple
problem to solve. Don't ever go to one.
