Default SHIPWRECK BLAMED ON BAGGY TROUSERS

A shipping accident that resulted in a massive clean-up and salvage
operation was apparently caused by a pair of baggy trousers.

"For the chief officer of the ship to knock himself unconscious from
catching his trousers on his chair, it shows just how easy it is for a
shipping disaster to occur with devastating consequences.

The report reads: "Here is yet another grounding and subsequent total
loss of a ship because the officer in charge of the watch became
incapacitated while alone on watch on the bridge, with the watch alarm
switched off. Whatever the cause of the OOW's (Officer of the Watch)
failure to alter course, the presence of a lookout on the bridge would
have prevented the grounding and loss of this ship.

"A side issue to this accident was the poor navigational practices on
board: neither the master nor the chief officer plotted any positions on
the chart after the ship had left port.
