I think I've got it now. You wear the farmer wetsuit, some sort of

Exactly! Although I wear a layer of that "sort of shirt that keeps you
warm and won't soak up water if it gets wet" under my wetsuit, it's not
there to prevent chafing (which my farmer johns which are cut for
paddling with extra wide openings for the arms) but more to provide a
more comfortable dry layer when I have to put on a damp wetsuit over it
on a winter day (for example after several days of paddling which make
all of my other dry wetsuits wet as well :-) ).

Though it's not quite as much of an issue with a farmer john/jane
wetsuit as it can be for a wetsuit with full length arms, wearing an
anti-chafing top underneath a wetsuit can be a good thing; especially
when you're going to be paddling for hours on end. You can usually
find something like this in surf shops.

I couldn't agree more! Having a well fitting wetsuit, paddling jacket
and swimming vest makes a huge difference in the comfort one has when
paddling for extended periods of time. I've had paddling clothes that
have rubbed my skin away after paddling in them for longer. Not much
fun, especially when you have to don those clothes again for the
remainder of a long paddling holiday.
