Personally, I think education should be mandatory, not to be chicken,
little, just because it is a largely unknown danger.
For example, carbon dioxide or methane gas can build up in a sewer or other
hole in the ground. If you jump in and take one breath, its like being at
50,000 feet, virtually no oxygen and you go down in seconds. The real
tragedy is when others jump in to save you. More people die trying to save
people in such incidents than those who enter.
Speaking of which, the best training would probably contain all such risks,
which fall generally under the title of "confined space training" which
includes poorly ventilated areas with large amounts of oxygen displacing
gases, such as behind a boat.

Also the exhaust fumes are heavier than air after coming out of a
watercooled exhaust, so will lie at water level, bit of a design fault
for a leisure boat...
