Navy Collision Report - better format
 

For those who tried to read the report that I posted previously,
here's another one in PDF format that's much easier to read:

Lots of blame to go around and some of the fails are almost
unbelievable for any experienced boater.

Interesting read. In the case of the Fitzgerald, it was at least
maintaining a constant course. It should have passed astern of the
Crystal however. Overall, a screw up for sure.

The appears the situation on the McCain was totally different. The CO's
order to transfer throttle control (only) to the Lee helmsman was
totally misunderstood. My uneducated guess is that it was the first
time any of the watchstanders had every heard of it.
